The Town Clerk files and issues certificates for all births and deaths that occur within Brookhaven Town. The Town Clerk also maintains marriage records for individuals that obtained their marriage license from the Brookhaven Town Clerk’s office.

You can also apply by mail or in person during normal business hours. The Town of Brookhaven may require additional documentation from the applicant prior to issuing vital records.
