Ordering Vital Records Online
The Town of Brookhaven Vital Records does not accept online orders; however, for your convenience, you can process online requests for birth, marriage and death records through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. Important Note: The Town of Brookhaven may require additional documentation from the applicant prior to issuing vital records.

Divorce Records
Divorce records are maintained by the County Clerk in the County where the divorce action was heard. You can contact the Suffolk County Clerk’s office at (631) 852-2000.

Legal Name Changes
Any Legal Name Change filed through the Suffolk County Special Term Division of the Supreme Court must be filed with the New York State Vital Statistics Office. The Brookhaven Town Clerk’s office will not have the amended document until the appropriate information has been submitted to New York State.

Puerto Rican Birth Certificates
If you are interested in obtaining a birth certificate from the Puerto Rican Government, information can be obtained from the Puerto Rico Federal Affairs Administration at (202) 778-0710 or send them an email at

As of November 1, 2010, the Town of Brookhaven no longer considers a Puerto Rican birth certificate valid if it was issued before July 1, 2010.

The Puerto Rican government, the U.S. Department of State and Department of Homeland Security collaborated in creating a new law on the island to match documentation policies in the United States, according to the government of Puerto Rico's website. The birth certificates had become widely used to obtained fraudulent identification and documentation in the United States, so the federal agencies and territorial government made changes to how the certificates are issued.

The following listing states the necessary requirements for the Town Clerks Office to accept the translation:

· The translation must be from a bona fide business, university, embassy, or consulate.

· The translation must include a statement of accuracy with the translator’s notarized signature. This statement must attest to the accuracy of the translation and that the translator is competent to translate the specific language.

· The translation must be on the company, school, university, or government agency letterhead.

· A photocopy of the original document from which the translation was translated must be attached.
